This release agreement is an understanding that Lutheran Hour Ministries (LHM) will be photographing and/or videotaping at the above named event, such that I, (the undersigned) hereby grant LHM permission to copyright and use the photographs/videos in any manner or form for any purpose lawful at any time, including the right to portray my name, likeness, and voice, and other reproductions of my likeness and voice. I waive any right that I may have to inspect or approve the finished product or written copy, that may be used in conjunction therewith, or the use to which it may be applied. You may edit my appearance as you see fit, and I release and discharge LHM from any liability by virtue of any distortion or alteration, intentional or otherwise that may occur or be produced, in the making or editing of the photos and/or video. I agree to hold LHM harmless from any liability to others arising from the use by LHM of anything said or done during the shoot. I understand that there will be no compensation for participation in the photo/video shoot. In consideration of my participation, I agree and understand that I assume the risks inherent in the filming and location, and with full knowledge of the risks, I agree to release and hold harmless LHM and its employees and assigned representative(s) from claims arising or related to my transportation or participation. I hereby accept all injury and risk to health that may result from such participation and release LHM, its employees and assigned representatives from any and all liability to me or for all and any claims and causes of action for loss of or damage to my property and for any and all illness or injury to my person, including death, that may result from or occur during my participation in the filming or means of transportation to the filming, whether caused by negligence of LHM, its employees or representatives or otherwise. I further agree to indemnify LHM, its employees and representatives from liability for the injury or death of any person(s) and damage to property that may result from my negligence or intentional act or omission while participating in the filming.

I understand and agree to abide by all rules and regulations established by LHM and its assigned representative(s) pertaining to the filming.

I consent to and give permission for emergency medical care that may be needed as a result of my participation.
